Rural Minnesota Primary Care and Community Team

Objective

The purpose of the Rural Minnesota Primary Care and Community Team: Opioids and Other Substances Project’s goals are to 1. Increase access to opioid use disorder treatment (Medication for Opioid Use Disorder and Substance Use Disorder Treatment) for individuals living in rural communities, who are Native Americans, and/or who are low income through an integrated primary care team, including physicians, advanced practitioners and care facilitation; Goal 2. Increase successful transitions between community organizations to better serve individuals living in rural communities, those who are Native Americans, and/or those who have low income; BS Goal 3. Successfully manage the grant including evaluation, dissemination of results, and sustainability of the project.
