Essentia Health adds community health worker to connect Iron Range patients with needed resources

Essentia Health knows that health care goes beyond the walls of its hospitals and clinics. That's why the health system employs the expertise of community health workers (CHWs). These professionals play a crucial role in improving health outcomes by helping people access care and connecting patients to important resources that address concerns related to food, transportation and housing.

CHWs have been an integral part of the Essentia Health care model since 2012. This year, Essentia identified a need on the Iron Range and is excited to announce that a CHW will serve the Virginia and Hibbing area to better support rural and underserved communities.

“As a community health worker, my primary goal is to support patients in achieving their goals,” said Brittni Abbett, who is serving in this position. “At the same time, we also support Essentia staff in delivering the comprehensive, whole-person care that they know is important.”

“By expanding this program, Essentia is strengthening local partnerships and advancing efforts to address social determinants of health,” said Emily Kuenstler, community health director at Essentia. “Community health workers play a vital role in advocating for patients, reducing health disparities and ensuring more people receive the care and support they need to thrive.”

CHWs are trusted members of the community who take the time to understand patients’ needs and goals. Their approach involves building strong relationships with patients and their families, understanding their unique needs and helping them navigate the health care system. Key responsibilities include building trusting relationships and setting personalized health goals. Providing tailored support in accessing health care, community connections and social support. Connecting patients to services through the Resourceful platform.

The impact of CHWs’ work is significant. They support more than 1,500 patients per year, utilizing an evidence-based model of care that has proven benefits in cost, health and patient satisfaction.

In 2024 alone, 220,000 patients completed screenings for social needs, with more than 26,000 patients reporting at least one need. Employing community health workers to provide hands-on support to help patients address these barriers is a key strategy for improving community health and well-being.
