Evaluation of the Ely Community Care Team, including the Behavioral Health Network

Objective

The purpose of this project, an embedded case study design, is to conduct both a process and outcome evaluation of the development of the Ely Community Care Team and interorganizational and patient outcomes. The project has two samples: 21 community organizations who comprise network and patients who receive care coordination.
