Your medical records document your medical information. They may include:
Trust Essentia Health to protect the confidentiality of your medical record. We’ll share your health information only as needed for treatment or payment, or with your permission.
You have the right to inspect, review, and receive a copy of your medical record.
Most of your Essentia Health medical records are available through our secure online patient portal, MyChart. Requested records are usually available within 2-3 business days for you to print or save electronically.
To request medical records by mail, fax or email, download, print, and complete our Authorization for Use and Disclosure of Protected Health Information [PDF]. You must sign the form by hand.

To grant Essentia Health permission to verbally discuss medical & billing information by mail, fax or email, download, print, and complete our Verbal Authorization Form [PDF]. You must sign the form by hand.
To forward your medical record information to Essentia from another health system, email to SendMyMedicalRecordstoHIS@essentiahealth.org
You’ll get coordinated care, even when you’re travelling and unable to visit an Essentia Health facility, thanks to our participation in Care Everywhere. This network allows participating health care organizations to quickly, instantly, and securely exchange medical records to help you get the care that’s right for you.
