The purpose of the Expansion of Evidence-Based Treatment for Opioid and other Substance Use and Misuse in Rural Primary Care Clinic Project is to expand access to office based opioid treatment (OBOT) and treatment for other SUDs in rural MN communities through evidence based strategies: integration of behavioral health (mental health and substance use disorders) in primary care; increased number of prescribers available to provide Medication for Opioid Use Disorder (MOUD); integrating care facilitation to coordinate referrals to treatment and recovery programs and to community organizations addressing social determinants of health; and RN care management into the rural primary care team; and development of pathways for transitions between clinics and other community organizations.
