LIFEteam: Providing Trauma Care with Community Support; Treating the Whole Patient


LIFEteam (Leadership Impacting Family Environment) is a hospital-based intervention program in Trauma Surgery that connects community resources and ambassadors with patients and their families affected by trauma. Essentia Health-St. Mary’s Medical Center (EH-SMMC) began offering LIFEteam consultations in 2018 to support patients with alcohol and substance abuse issues, at risk youth, victims of violent crime, and those needing additional social services. The goal of this study is to perform a retrospective review of patients who have received a LIFEteam consultation at EH-SMMC to demonstrate the feasibility of incorporating community ambassadors into hospital care. Post-discharge data to be assessed includes: Hospital re-admission up to one year after a LIFEteam intervention; drug and alcohol screening labs at any hospital re-admission; treatment for substance use and mental health issues; support group attendance; geriatrics recurrent falls and care transition plans; and education attendance, positive activities, or employment for at-risk youth. The investigators hypothesize that LIFEteam consultations provide support that will lead to trauma prevention, decreased hospital re-admissions, and improved social welfare.
