STRIDE (Strategies to Reduce Injuries and Develop Confidence in Elders) is a large national study ( funded by the National Institute on Aging and PCORI (Patient Centered Outcomes Research Institute).  Led by physician investigators at Harvard, Yale, and UCLA, this study aims to test the effectiveness of a comprehensive care management model (including risk assessment, patient engagement and care coordination) to reduce fall-related injuries among older adults.  Essentia is one of 10 healthcare systems selected to participate, with eight of our family medicine clinics engaged in the project.  The Essentia research team includes Allise Taran, Erica Chopskie, Margaret Hoberg, Siobhan MacMahon (Co-PI), Paul Hitz, Lisa Wilkinson, and Steve Westholm.
