“Advance care planning is not simply filling out an advance care directive. ACP is a process across time of understanding, reflecting on and discussing future medical decision, including end of life decisions. ACP is the best way we at Essentia have found to understand the goals, values and beliefs of an individual and incorporate what matters most to each person into the care they receive.” Says Wanda Paulsen, RN Advance Care Planning Program Manager.
Advance Care Planning (ACP) is a process of understanding, reflecting on and discussing what matters most to you and helps prepare those closest to you for a time if ever you couldn’t speak for yourself. ACP helps you have a say in your healthcare and make sure your goals and wishes for medical care can be known, honored and incorporated into the care you receive.
ACP also helps you to create a written plan (healthcare directive), to follow your wishes. This document allows you to name who you would want to make medical decisions for if ever you couldn’t speak for yourself.
