Graceville, Minnesota—just the name gives you a sense of what it’s like to live here. And at Grace Home, a 45-bed long-term senior care center that’s been part of this rural community since 1976, we share the same small-town values our residents grew up with. We’re lifelong friends and good neighbors who, with a foundation in faith, care about each other, not just for each other. We’re just across the road from Lake Toqua, surrounded by sweeping, central-Minnesota farm-scapes. This serene setting offers the comfort and security our residents deserve.
Grace Home is clean, warm and welcoming, and in addition to our double rooms, you’ll find 14 private suites, six semi-private rooms and 24-hour skilled nursing care. Mostly what you’ll find, though, is compassion and camaraderie. We build strong bonds with the people who live here and work side-by-side with them to ensure every need is met. As one of our residents put it: “If I can’t be home, I want to be here.”

Grace Home was built in 1976 as a 60-bed, skilled nursing senior home by the City of Graceville and sits adjacent to Holy Trinity Hospital and Graceville Clinic. The City contracted with the Franciscan Sisters of Rochester Minnesota, to manage the facility, and when city officials made the decision to withdraw from Grace Home, the Missionary Benedictine Sisters (who owned Holy Trinity Hospital) agreed to take over management. In June of 1996, the sisters purchased Grace Home from the City and continued to own and operate it until it was acquired by Essentia Health in October, 2010. Over the years, Grace Home has adapted to changing times and needs by transitioning into a 50-bed facility with more private rooms, thus meeting senior preferences for greater privacy and comfort.
